Major Responsibilities:
To deliver and coordinate social work services for public health clients including services to individuals, families, groups and communities. Services may include but are not limited to services for individuals with communicable diseases such as Tuberculosis or Sexually Transmitted infections including HIV/AIDS.
- Provides social work services for public health clients including individuals, families, groups and the community according to the program specific policies and procedures and operational plans of TPH and the Standards of the Ontario College of Social Workers and Social Service Workers.
- Carries out work in various community locations as required including but not limited to client homes, shelters, community agencies, TPH offices and schools.
- Makes initial service response assessment; provides information, education and counselling and makes referrals on issues surrounding socio-economic conditions, immigration and settlement, financial resources and planning, employment and educational attainment.
- Carries out independent home visits, assesses individual/family status and assists the individual/family in developing and achieving goals related to the determinants of health and/or communicable disease control (e.g., the successful completion of TB treatment).
- Provides therapeutic counselling to clients and/or their families who are experiencing challenging life situations such as addictions, sexual abuse, mental health, fetal alcohol spectrum disorder, violence against women and homelessness.
- Communicates effectively with high risk clients and their families.
- Refers clients to appropriate community resources and other professional services and assists and supports clients to access these services.
- Provides crisis intervention e.g. urgent housing/financial issues etc as required. Advises/reports to appropriate authorities (e.g., Children’s Aid Society, Associate Medical Officer of Health) as required and within the bounds of related legislation.
- Maintains individual/family/group records and necessary documentation according to the policies and procedures of TPH as well as the Standards of Practice set out by the Ontario College of Social Workers and Social Service Workers.
- Collaborates and consults with community groups, coalitions and agencies such as schools, community centres or hospitals
- Facilitates access to TPH program and services.
- Provides consultation on policy development and implementation based on appropriate legislation and best practices.
- Assists clients to complete appropriate documentation to obtain necessary assistance e.g. financial, housing, immigration etc.
- Advocates on behalf of clients and/or their families.
- Provides language interpretation when possible for clients to facilitate accessing of care and links clients to interpretation services as required.
- Provides consultation related to client’s social issues to team members, management and other service providers.
- Participates in case conference meetings to facilitate coordination of client care.
- Develops and maintains an up to date community resource manual as directed.
- Liaises with community partners to collaborate on client care issues.
- Mentors and provides orientation and training to new staff and students.
- Provides educational workshops within the community and to internal staff as appropriate.
- Maintains statistics. Prepares and submits activity and other reports as required.
- Appears and gives evidence in court and inquests when subpoenaed in consultation with management and legal services.
- Presents and participates in professional conferences and workshops. Makes presentations to other health professionals/ colleagues.
- Participates in the development of internal policies, procedures and protocols and updates resources for managerial approval.
- Promotes the implementation of municipal, provincial and federal legislation and strategies.
- Supports TPH response to emergencies requiring Public Health assistance.
- Participates on internal and external committees as directed.
- Assists in the development of new and ongoing programs and services.
- Participates in program evaluation.
- Identifies and establishes contacts with community groups, agencies, businesses and individual community leaders.
- Works as part of the applicable multidisciplinary team.
Key Qualifications:
Bachelor of Social Work (B.S.W.) and current registration with the Ontario College of Social Workers and Social Service Workers.
Community development experience.
Health promotion experience and experience in working in an outreach capacity.
Experience in the provision of counselling related to social, health, settlement and housing issues.
Experience working with new immigrants and refugees and in dealing with settlement issues.
Experience working with challenging and high risk client situations e.g., clients dealing with addictions, individuals involved in the sex trade, victims of sexual abuse, mental health issues, violence against women and homeless, street involved individuals.
Must possess a valid Ontario Driver’s Class “G” License and have access to a vehicle.
Excellent assessment and counselling skills.
An understanding of the broad system perspective with knowledge of and ability to access community and social service agencies.
An understanding of beliefs and value systems of others and demonstrated ability to conduct work in a non-judgemental manner.
Excellent interpersonal skills and the ability to handle challenging assignments in a high profile professional and political environment.
Flexibility to meet competing demands and respond to client needs as they arise.
Ability to work independently in a community setting.
Ability to work as a member of a multi-disciplinary team.
Ability to communicate clearly and effectively both orally and in writing.
Competence with software applications including Microsoft Word.
Fluent language skills in languages other than English would be an asset.
